- The medical report made will be sent to your email (that you provide) and if you authorize the referring doctor.
- Establish communication between the referring physician and medical personnel to promote and optimize the care of the pediatric patient.
- Preparation of receipts of fees to comply with tax provisions.
For the above, we require to obtain the following personal data:
- Patient’s full name
- Age
- Date of birth
- Parents name
- Place of residence and contact telephone number
- Background
- Symptom
- Vital signs
- Cabinet studies: radiography, electrocardiogram and echocardiogram
